The ongoing worldwide Coronavirus (COVID-19) pandemic now has thoroughly documented risks. Our office is following CDC guidelines for conducting in-person sessions to the best of our ability. Though California State rules for wearing masks indoors may change, Evguenia Razinkova, MT, and ATLG will continue to require masks to be worn in all communal spaces of our office to minimize the spread of COVID-19. Additionally, as of June 2021, all patients who come for in-person sessions must be fully-vaccinated with either the Pfizer, Moderna, or J&J COVID-19 vaccine. Because the CDC has deemed that fully-vaccinated individuals can meet indoors without masks, should you and I mutually decide at any time during our session to remove our masks, we each accept full responsibility for that decision.

After carefully considering all the potential risks involved, I hereby assume the same and agree to release and hold-harmless Evguenia Razinkova, MT and ATLG, and all staff who practice at 800 Pollard Rd., Suite B201, Los Gatos, Ca 95032 from and against, all claims and liability resulting from exposure to disease-causing organisms like COVID-19.
